A Pennsylvania-based healthcare provider is notifying millions of patients about a data breach that exposed their personal and health information.

Data from the U.S. Department of Health and Human Services shows that AdaptHealth suffered a hacking/IT incident, impacting 4,115,802 individuals.

According to the firm, an unauthorized entity gained access to its cloud-based systems and stole files that may include names, contact information, demographic information, health insurance information, and health information. The firm highlights that the files did not include Social Security numbers, financial information, including credit or debit card information, or bank account information.

“AdaptHealth, LLC (‘AdaptHealth’) has announced that on June 15, 2026, AdaptHealth discovered it was targeted by a cyberattack on June 5 that led to unauthorized access to certain company systems that contained personal information. Once the incident was discovered, AdaptHealth immediately took steps to stop the attack, notify law enforcement, and initiate a thorough investigation of the incident.”
